1. If you are unable to print out a Band Profile Order Form please click here to email us with your mailing address and we will send you a form.
2. Include write-up.  Make sure you do not go over the maximum number of words for the size that you purchase.  You do not need to count the words for the contact information.  That will automatically be added to your ad.
3. Please send a real photo.  Computer printouts do not duplicate well.
